If you have been asked to be a Godparent or Confirmation Sponsor for someone and are in need of what is known as an eligibility letter, you can receive one by stopping by the rectory during regular hours or by filling out the form below.
Please note that you must be a registered parishioner here at OLMC in order for receive one. Verification will take place prior to the letter being completed.
